Community Connectors in Lincolnshire

Every lot, on one map

The Community Connector service in Lincolnshire, commissioned by NHS Lincolnshire ICB, is split into twelve lots, each run by a local organisation. Bridge runs four of them: Lincoln North, Lincoln City, South Lincoln and Sleaford.

Boundaries are approximate. They follow council wards, matched by eye to the ICB's lot map, so a few wards on the edges are a best guess. Lot names and numbers are from the ICB's 2026 service specification.
